What we collect

When you submit a form on this site to join the evaluation program, request a technical brief, or inquire about partnerships, we collect:

  • Your name
  • Your work email address
  • Your organization or facility name
  • Your professional role (surgeon, researcher, educator, etc.)
  • Your stated interest in Silica Medical (evaluation, advisory, technical brief, etc.)
  • Any optional notes you provide about your interest or application
  • The timestamp of your submission and your browser's user agent string

How we use this information

We use the information you provide to:

  • Respond to your inquiry about the evaluation program or technical information
  • Assess fit for participation in our research evaluation or surgeon advisory programs
  • Schedule follow-up conversations or send requested materials
  • Improve our understanding of the needs and interests of surgeons, researchers, and institutions

Where we store your data

Your submission is stored in Cloudflare Workers KV, which serves as our system of record. If you provide consent, we may also forward your information to a Google Sheet via an Apps Script webhook for internal operations tracking. Cloudflare's infrastructure is located in multiple geographic regions; your data may be replicated for redundancy.

How long we keep your data

We retain your submission for up to 2 years to track evaluation participation, advisory board engagement, and pilot results. You can request deletion at any time using the contact form on our homepage.

What we do NOT collect

We do not collect:

  • Phone numbers (except if you volunteer them in the notes field)
  • Patient information, health records, or medical data
  • Credit card or payment information
  • Confidential research protocols, formulas, or proprietary surgical techniques

Spam protection

All forms are protected by Cloudflare Turnstile, which verifies that you are human. Turnstile does not track you across sites and does not require cookies.

Analytics

We use Cloudflare Web Analytics, a privacy-first analytics tool that does not require consent banners and does not use cookies to track you. It provides us with high-level traffic and page-performance data only.

Your rights

You have the right to:

  • Request access to your data
  • Request deletion of your data
  • Correct any inaccurate information
